This form to be completed online for each non-District owned personal computer connected to a San Antonio ISD local area network (LAN). This form is online at the SAISD HelpDesk web page

Purpose: Use this form to request authorization to connect a non-District-owned computer or other device to a District local area network (LAN).

Processing: Once this electronic form is completed, the campus/department administrator may be contacted. Each campus and/or department is responsible for ensuring this policy is adhered to. Electronic forms will be kept on file indefinitely.
